For your convenience, you can complete your new patient paperwork online. You will need to sign in with your last name, date of birth in DD/MM/YYYY format and a PIN from our office. Ask for your PIN number when you call to make your first appointment. Click below to take you to the patient portal. (Most common temporary PIN is 1111)
NOTICE When filling out your paperwork be sure to save each step and click next. Type your name in the box provided and click affirm on consent forms.
Your first visit
You will receive a very thorough and comprehensive consultation/history and exam. Dr. Pray will discuss the treatments available based on your exam findings. Most patients will receive an adjustment and several therapies. If x-rays are needed there will be a convenient referral made.
